There are really so many ways to take this. I'm sure you'll come up with the perfect thing. I mean, if an injury is THAT bad, John might have no choice but to take Sam to a hospital . Or perhaps they go to the fruit stand and someone accidentally sees something. As for Dean, I think it depends. It would take a lot to get him to step in, you're right.
