The funnydark thing about that whole conversation, is that even though it's NOT John, in that it's that monster thing saying this...well, it's interesting to consider WHERE the MOW got these ideas of what to say from. Is he pulling them from Dean's brain? This seems likely, because even before there were phones, the MOW would wait in the woods and connect somehow with his victims. So, he's connecting with Dean and he's telling him EXACTLY what he wants to hear.
So, the whole "I love you, you're my boy" is what Dean wants and needs to hear. Since if someone were listening in to the conversation, we know that they wouldn't hear the MOW. Only the victim does. So the conversation is rather in the victim's head.
As to why Dean needs to hear "You're my boy," well then. That's where the daddycest comes in. I think, "You're my boy" has a WHOLE lot more daddycest implications than "I love you, boy," (though I still like the idea of him saying that....) Don't most moms and dads say "You're my child"? I mean, would a mom or a dad say, "You're my girl?"
And you know...I think that John is the only one Dean's ever said, "I'm sorry" to. Like John's the only one Dean will submit to. (Oh, did I just use the word submit in an un-sexual way. That's a new one!) And Dean saying I'm sorry...okay, I can totally see it now, thank you. It's been over a year since Dad died, and he reverts to son mode so fast...Hey, Dean, just line up for the spanking and get it over with while we watch.
