I understand where Dean is coming from. He's ashamed and 'stuck' in a situation that is pretty much a 'no win'...1) Staying there with Sammy until he finds out in fear that Sam will hate him or 2) Leaving Sammy and still worrying about him..how can he not?? He has a HUGE problem with abandonment issues and he fears that Sam will leave him once he finds out due to their past..
With that said, I couldn't imagine being Sam in this situation...if Dean leaves...Sam is LITERALLY all alone with no memory of who he is and all his love and devotion is wrapped up in Dean...Dean is his lifeline...
